Google Forms and Qualtrics
Google Forms and Qualtrics are survey platforms that are available for free to members of the Portland State University community. Either tool can be used to create and distribute surveys, analyze survey results, and share either with classmates, colleagues, and other members of the PSU community.
Using Google Forms
To access Google Forms, navigate to docs.google.com/forms in any web browser. Sign in with your Odin account when prompted.
Create a Survey with Google Forms
Navigate to docs.google.com/forms.
In the Start a new form area, select a form template or select Blank form to build your own.
Enter a name and description for your form.
Build your form by adding accessible question types and selecting a high contrast color palette.

Using Qualtrics
To access Qualtrics, navigate to portlandstate.qualtrics.com in any web browser. Sign in with your Odin account when prompted.
Create a Survey with Qualtrics
Navigate to portlandstate.qualtrics.com.
In the Welcome to XM column, under Recently visited, select Create a new project.
Select Survey.
Build your survey by adding accessible question types and selecting a high contrast color palette.
