Create Surveys with Google Forms or Qualtrics

Qualtrics and Google Forms are free survey creation tools available to the Portland State community.

Qualtrics

Qualtrics helps you collect and analyze survey data. This software has advanced features for experienced survey designers.

Create a Survey with Qualtrics

  1. Go to Qualtrics at portlandstate.qualtrics.com.

  2. If prompted, log in using your Odin information.

  3. If you haven't used Qualtrics before, accept Terms of Service by selecting the “I Accept” button.

  4. Create a Qualtrics Project to store your survey. Select the New Project option from the Qualtrics home screen to create a new project. For more details on project creation, visit Qualtrics Creating a Project article

  5. Once your project’s been created, you’ll be taken to the Survey Builder for the project, which you can use to create and publish your survey. This help article provides more information on creating surveys.

Google Forms

Google Forms are a quick, easy way to create surveys.

Create a Survey with Google Forms

  1. Go to drive.pdx.edu.

  2. Select the New button > More > Google Forms.

  3. Enter a name for your Untitled Form.

  4. Build your form by adding questions, images and selecting a theme/color palette.
